  1. Barangay Business Clearance
  2. Certification From Barangay (Presence of Segregated Waste Cans)
  3. Previous Laminated Mayor’s Permit
  4. Copies of the Annual or Quarterly Tax Payments
  5. Audited Financial Statements
  6. BIR Registration Cetificate
  7. Declaration of Previous Year’s Gross Sales
  8. 1 pc. 1 1/2 x 1 1/2 ID Picture or Logo
  9. Certificate of Tax Exemption from local Taxes of Fees, If exempts

Clearances

  1. Zoning Clearance, Engineering Clearance, Tax Clearance
  2. MENRO Clearance
  3. Police Clearance
  4. Sanitary Permit, Health Certificate
  5. Fire Clearance

B. To One Stop Shop for Assessment

C. To Treasurer’s Office for Payment of Taxes/Fees

D. To Fire Office for Fire Safety Inspection Certificate

E. Back to One Stop Shop for Issuance of Laminated Business Permit
